The FACTS research projects operate under approval from the Institutional Review Board, an administrative body established to protect the rights and welfare of human research subjects recruited to participate in research activities. In order to comply with our IRB requirements, consent forms are required for all individuals involved in the study (including students, teachers, and administrators). There are specific consent forms for varying levels of participation in this research. Please use the links below to access these forms if you are a participating school in FACTS.
For standard participation in FACTS (click on the links to access the Word files):
Teacher consent- for teachers administering pre and post assessments, using curriculum materials, copying student work for researchers, and participating in or having students (needed for all school-based participation).
Administrator consent- for administrators who are interviewed about how they support teachers in their school who are using innovative teaching materials (needed for all school-based participation).
Parent consent: Information letter and consent for parent with a child participating in study where identity of child is known to researchers (needed for school-based participation that involves student data collection).
Student assent: Student assent where identity is known to researchers (needed for school-based participation that involves student data collection).
Administrator permission letter- for administrators who have teachers and students participating in more inclusive data collection where student identity may be known to researchers (needed for school-based participation that involves student data collection).

Adult Non-student Consent - for non-school data collection among legal adults that is compensated.
